Schedule:

Standard operating hours are 8:00 am – 4:30 pm; Monday-Friday. Please note work hours may fluctuate depending on the Participant and business needs, flexibility is required.

Hourly Wage:

$29.00 /hr

Job Summary:

At LIFE-NWPA, our Master Social Workers are responsible for the direct social work case management services to LIFE-NWPA Participants. Provides social services support, including Participant screening, case management, counseling, and referral.

Duties/Responsibilities:

  • Participate in the Interdisciplinary Team’s initial assessments, care planning, and ongoing re-evaluations of the Participant’s case.

  • Maintain current written case management records, including periodic reassessment of program Participants.

  • Provide individual and family counseling; develop and lead group counseling and activities.

  • Refer Participants to appropriate community agencies or facilities, acts as a liaison with such organizations and as an advocate for Participants.

  • Create effective relationships among staff geared to building and maintaining a cohesive team.

  • Consult with and advise staff members on the relationship of social, emotional, and cultural factors to health and medical care and the availability of social services in the community.

  • Participate in program and policy development of the social work component of the program.

  • Communicate Participant changes to team members.

  • Participate in on-going Quality Initiative training as directed.

  • Other duties as assigned.

Required Skills/Abilities:

  • Must possess the ability to make independent decisions when circumstances warrant such action.

  • Must be able to deal tactfully with personnel, Participants, family members, visitors, government agencies/personnel, and the public.

  • Ability to organize work and follow through on assigned tasks.

  • Excellent written and verbal communication skills.

  • Must have the cognitive ability to process and perform tasks for Participant safety.

  • Attends and participates in continuing educational programs.

  • CPR and First Aid Certification.

Education and Experience:

  • Master’s Degree from an accredited School of Social Work (MSW) with one year of experience working with the frail/elderly population.

  • One year of experience working with the frail and elderly population or willing to participate in extensive training as needed.

  • This position requires a valid state driver’s license and reliable transportation.

Requirements:

  • Requires valid State driver’s license with a clean driving record.

  • Successful completion of background check, physical, drug screening, TB Screening

Physical Requirements:

  • Works inside the facility throughout the Participant and office areas.

  • Sits, stands, bends, lifts, and move intermittently during working hours.

  • Is subject to frequent interruptions.

  • Must be alert for Participant’s safety.

  • Is subject to lifting, carrying, and supporting Participants.
