Direct Care Supervisor

Schedule:

Standard operating hours are 8:00 am – 5:00 pm; Monday-Friday. Specific work hours will be established through discussions with your supervisor.

Job Summary:

Supervises and coordinates all in-center and home care Personal Care Aides (PCA) to ensure participant care activities are aligned with their plan of care.

 

Duties/Responsibilities:

  • Ensures center and home care PCA operations and environment are within audit standards, including completion of all required logs.
  • Supervision of in-center and home care PCAs.
  • Will attend and participate in all IDT Meetings.
  • Responsible for the master schedule, center, and home care PCA schedules.
  • Responsible for ensuring Participants care planned interventions are being completed as pertained to their scope of care.
  • Ensures that participants are receiving appropriate supplies per their needs by being involved in the coordination of the delivery of supplies with appropriate departments.
  • Ordering appropriate durable medical equipment and completing authorizations as needed.
  • Complete necessary documentation and encounters in the EMR in a timely manner.
  • Review documentation and encounters in the EMR in a timely manner.
  • Responsible for completion of audits, including but not limited to home care PCA documentation
  • Responsible for completing initial and annual competencies for the Direct Care Aide Scheduler, all in-center and home care PCAs.
  • Responsible for reviewing, verifying, and approving electronic timesheets for the Direct Care Aide Scheduler and all PCA payroll, mileage, and expense reimbursements.
  • Completes all initial, six months and significant change assessments in the home to assess for participants resources, safety, functionality and living environment.
  • Assist with developing a plan of care based on the in center or home care need and which incorporates the plans of other disciplines and continuing care needs; includes the participant/family in developing or revising the plan of care; makes referrals to multidisciplinary support services.
  • Participate in, and complete on-going training as directed.
  • Monthly supervisory visits with PCAs.
  • Participate in on-going Quality Initiative training as directed.
  • Other duties as assigned.

Education and Experience:

  • Bachelor's degree
  • One year working with the frail/elderly population
  • 5+ years' experience in:
  • o Supervision, scheduling, Pacelogic, and Prime Suite
  • Microsoft Office (intermediate to advanced)
  • Valid driver’s license
  • CPR and first aid certification

 

Requirements:

  • Requires valid State driver’s license with a clean driving record.

  • Successful completion of background check, physical, drug screening, TB Screening

 

Physical Requirements:

  • Must be able to move intermittently throughout the workday.
  • Must be able to lift, push, pull, and move a minimum of 25 pounds.
  • Must be able to assist with the evacuation of Participants.

Employment Requirements: Successful completion of Pennsylvania Criminal History Check, PA DHS Child Abuse Clearance, Office of Inspector General check, and FBI Fingerprinting for Child Protective Services Law (at employer’s expense). Successful completion of post-offer employment drug screening and physical (at employer’s expense). CPR and First Aid Certification, or willing to obtain at employer’s expense. Individuals must have one year of prior experience caring for the frail elderly or willing to complete Company paid training to meet this requirement.


This position has full access to Participant, program, and staff records and reports, requiring discretion to protect individuals and the program.
