Schedule:
Standard operating hours are 8:00 am – 5:00 pm; Monday-Friday. Specific work hours will be established through discussions with your supervisor.

Job Summary:
Under the general direction of the Transportation Manager, plans, organizes and implements the transportation schedules of the Center.
Duties/Responsibilities:
Coordinates the activity of staff entering private homes of LIFE-NWPA Participants.
Supervises the daily activities of the Drivers/Aides.
Responsible for coordinating all outside transportation service providers and logging transportation encounters into Pacelogic.
Ensures that Drivers complete pre-trip inspections and collect gas receipts to be processed.
Utilizing Tripmaster Scheduling Software:
Prepares daily transport schedules.
Communicates changes in Participant status to drivers.
Communicates specific Participant transport needs to drivers.
Maintains an accurate Participant list.
Properly assigns automated reminder calls that go out to Participants and/or participant caregivers.
Coordinates appointments, and medication/supply deliveries, often at the request of Clinic staff.
Communicates vehicle maintenance issues to Transportation Manager.
Schedules vehicle maintenance appointments at the direction of the Transportation Manager
Submits maintenance invoices to Transportation Manager for tracking and payment.
Conducts initial and periodic assessments.
Participates as a member of the multidisciplinary team.
Participates in the development of the IDT Plan of Care.
Completes all documentation in electronic medical records as required.
Monitors Driver’s attendance within the company’s electronic attendance tracking system, approves Paid Time Off Requests with the Center Manager and ensures any necessary payroll information is submitted timely.
Transport Participants when needed.
Participate in on-going Quality Initiative training as directed.
Other duties as requested/assigned.
Supervisory Responsibilities:
Provides supervisory oversight for assigned Center Drivers.
Required Skills/Abilities:
Must have access to reliable and consistent transportation.
Must be able to deal tactfully and patiently with personnel, participants, family members, visitors, government agencies/personnel, and the public.
Communicates with the medical staff, nursing personnel, and other department supervisors.
Ability to organize work and follow through on assigned tasks.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must have excellent written and verbal communication skills.
CPR and First Aid Certification.
Education and Experience:
High school diploma or equivalent is preferred.
1 to 3 years of relevant work experience.
One year of experience working with the frail and elderly population or willing to participate in extensive training as needed.

Physical Requirements:
Sits, stands, bends, lifts, and moves intermittently during working hours.
Is subject to frequent interruptions.
Is subject to lifting, carrying, and supporting Participants.
