Case Manager (RN)

Location: This position will support our LIFE centers in Northwestern PA.

Schedule:

Standard operating hours are 8:00 am – 5:00 pm; Monday-Friday. Specific work hours will be established through discussions with your supervisor.

Hourly Wage:

$38 / hr.

Job Summary:

A Case Manager is responsible for coordinating and facilitating various services to clients by assessing, planning, and implementing steps to improve their overall wellbeing. Discharge planning is the process of predicting the participants’ continuing medical, physical and emotional needs after they leave the acute or skilled facility and putting in place a plan to meet those needs.

Duties/Responsibilities:

  • Work Collaboratively with internal and external partners to meet participant needs
  • Assess participants needs and develop individualized plans of care
  • Conduct assessments to determine patient needs for resources
  • Identify and address social determinants of health that may impact patient well-being
  • Collaborate with healthcare teams
  • Participate in participant care conferences
  • Coordinating healthcare services, appointments, and referrals
  • Coordinating care transitions and discharge planning
  • Coordination of complex cases
  • Providing participant education on treatment options, self-care, and preventive measures
  • Ensuring accurate and up-to-date documentation of participant information and plans of care
  • Monitoring and evaluating participant progress and outcomes
  • Managing and coordinating medication adherence and reconciliation
  • Managing, ordering, and coordinating durable medical equipment (DME) patient needs as needed.
  • Assisting with insurance authorization as needed
  • Monitoring and managing healthcare length of stays
  • Advocating for participants

Required Skills/Abilities:

  • Relates well with people of all ages but has a deep sensitivity and commitment to the elderly
  • Ability to organize work and follow through on assigned tasks
  • Strong verbal communication skills and can follow written and/or follow oral direction
  • Must possess the ability to make independent decisions when circumstances warrant such action
  • Must be able to deal tactfully and patiently with personnel, participants, family members, visitors, government agencies/personnel, and the public.
  • Critical thinking, problem-solving and decision-making skills are essential for assessing participants’ needs and evaluating options and solutions.
  • Organizational, time management and multitasking skills are needed to manage multiple cases, tasks, deadlines and priorities.
  • Conduct Participant Assessment and evaluation for care planning and coordination of care
  • Possess skills in Resource management and allocation
  • Demonstrates cultural competency and diversity awareness
  • Demonstrated the ability to collaborate with healthcare teams to discuss participant progress and deliberate on appropriate care
  • Must possess the ability to be adaptable and flexible
  • Analytical skills to review documented performance for input into participant care plan.
  • Aligning case management strategies with well-being of individuals.

Education and Experience

  • Registered nurse in the State of Pennsylvania
  • At least three years of experience working as a registered nurse.
  • Preferred bachelor’s degree in nursing
  • Minimum of three years of case management experience
  • Preferred Case Management Certification
  • One year of experience working with the frail and elderly population or willing to complete Company paid training to meet this requirement
  • Must possess a valid PA driver’s license, vehicle, and clean driving record
  • Good interpersonal skills and the ability to work with geriatric patients and handle unusual interpersonal situations

Requirements:

  • Requires valid State driver’s license with a clean driving record.

  • Successful completion of background check, physical, drug screening, TB Screening

Physical Requirements:

  • Must be able to move intermittently throughout the workday
  • Must be able to lift, push, pull, and move a minimum of 25 pounds
  • Must be able to assist with the evacuation of Participants
  • This position may encounter issues/infestations in the homes of the Participants. Following proper protocols is essential.

Employment Requirements: Successful completion of Pennsylvania Criminal History Check, PA DHS Child Abuse Clearance, Office of Inspector General check, and FBI Fingerprinting for Child Protective Services Law (at employer’s expense). Successful completion of post-offer employment drug screening and physical (at employer’s expense). CPR and First Aid Certification, or willing to obtain at employer’s expense. Individuals must have one year of prior experience caring for the frail elderly or willing to complete Company paid training to meet this requirement.
